Vancomycin Trough
Useful For
Monitoring levels of vancomycin to ensure appropriate therapy. Aminoglycoside monitoring requires both peak and trough levels.

Specimen Requirements
Container/Tube: Plain, red-top tube(s)
Specimen Volume: 1 mL of serum
Collection Instructions: Serum for a trough
level should be drawn immediately prior to next dose. Initial
trough level of aminoglycoside will be drawn before the fourth
dose.
Note: 1. Random levels are drawn on dialysis
patients.
2. Dosage information, and which other antimicrobials are given
concurrently, and date and time of draw are required on request
form for processing.
3. Protect sample from light

Room Temperature up to 2 hours
Refrigerated up to 3 days
Frozen up to 14 days

Enzyme Immunoassay.
Reference Values
Therapeutic concentration: 5-20 mcg/mL
Toxic concentration (automatic call-back): >30 mcg/mL
NOTE: To convert mcg/mL vancomycin to umol/L vancomycin, multiply the result by 0.67
To convert mcg/mL vancomycin to mg/L vancomycin, multiple the result by 1.0
